San Jose, CA – Medical Examiner’s Office Identifies Motorcyclist Killed during Fatal Accident on McLaughlin Avenue near Panoche Avenue
San Jose, CA (August 15, 2018) – Recently, local news sources provided updated information concerning the motorcyclist who died as the result of a fatal accident that took place on McLaughlin Avenue, in the vicinity of Panoche Avenue. The tragic incident occurred at approximately 7:35 AM in the morning on Tuesday, August 14, 2018. As of today, the Santa Clara Medical Examiner’s Office identified the victim as 35-year old Andrea Henderson, of San Jose.
According to the original report, Henderson had been riding a 2009 Kawasaki motorcycle southbound along McLaughlin. At the same time, 23-year old Daniel Douglas Tapia had been driving a Honda Accord driving along the right lane on the same route, adjacent to the bike. At one point, Tapia attempted to execute a U-turn onto Panoche Avenue when, without warning, he plowed into Henderson’s motorcycle.
Officials confirmed Henderson was brought to a nearby hospital, where she succumbed to her injuries and died sometime later.
Afterward, Tapia remained at the crash site and spoke with law enforcement officials. Later, police arrested the motorist for operating a vehicle without a valid license and vehicular manslaughter and sent him to Santa Clara County Jail.

Based on the information provided by the authorities, Mr. Daniel Douglas Tapia will be held fully responsible for causing the violent accident and inflicting fatal injuries on the motorcyclist. Police have detained him for an act of recklessness and operating a vehicle without a valid license. As part of the investigation, officials will also consider if the motorist consumed drugs or alcohol prior to driving.
